Taiga 1015: Fix sphinx warning messages when HTML generated

Fixes most warning and undefined label messages when generating HTML
using Sphinx. This mainly fixes broken links and cross references in the
HTML output, or tidies up the source files

Changes made include:

- Add labels for the main heading at the start of all files that don't
have them
- Add labels for other headings that did not have them, where identified
- Replaced characters in labels / with -, & with and
- In system preferences, labelled major section headings as
_heading-system-preferences-label, rather than just label-label
- Replace & with 'and' in headings, labels, and text unless it matches
with the Koha interface or is part of an organisation's name
- Fix references formatted incorrectly
- Remove extra blank lines not required
- cron_jobs.rst: add cron- at the start of all labels so they are unique
- AllowRenewalOnHoldOverride system preference was indented and was not
displaying in output (new preference introduced in 19.05), added
description from release notes
- faq.rst: add faq- at the start of all labels so they are unique
- Update links to schema.koha-community.org to the correct URLs
- Update Baked & Taylor website URL
- XISBN services system preference was removed in Koha 18.11: removed
preferences OCLCAffiliateID, XISBN and XISBNDailyLimit (bug 21226),
removed cron job misc/cronjobs/services_throttle.pl (bug 21235)
- Use direct reference to system preferences rather than using a label,
unless it needs to be shortened, has \_ in the title,  or there are
multiple preferences in the heading
- Minor edits when identified

Tidy up source files to:
- remove trailing spaces at the end of lines
- remove extra blank lines
- fix any :ref: links where there needs to be a space between the text
and the angle bracket for the start of the label

system preference. This enhancement was added in Koha 18.11 and allows
Indian libraries to add their Amazon Affiliate ID and benefit from the
Amazon affiliate program.

Taiga 778 (8630): Add Adlibris cover images system preferences

Updates System preferences > Enhanced content with the Adlibris system
preferences. This enhancement was added in Koha 18.11 and allows
libraries to use cover images from Swedish book retailer Adlibris.

Taiga 779 (14222): (DM follow-up) Update OpacShowHoldQueueDetails system preference

Now that master is the 19.11 version of the manual, update the version
number in index.rst and conf.py from 19.05 => 19.11.

The version number in index.rst now uses a variable, so only conf.py

Reverse change made for System preferences > OPAC > Appearance > OpacShowRecentComments. With 'commentsettings' the sentence does not read very well, looks like the word was accidentally pasted.
