Taiga 995: Update version number

Taiga 995: Update version number

Now that master is the 19.11 version of the manual, update the version
number in index.rst and conf.py from 19.05 => 19.11.

The version number in index.rst now uses a variable, so only conf.py
requires updating for future versions.
